What
Hospital Council, along with CHA, has been meeting with member hospitals in the Fresno/Madera and Tulare/Kings sections to learn more about the barriers to discharging patients to lower levels of care. In some cases, patients waiting for transfer can occupy as many as 10% of the available beds in a general acute care hospital (GACH). The work is in the beginning stages, and further action is being explored to determine if there are ways to help GACHs discharge to the appropriate care level.
Take Aways / Next Steps
The meetings are ongoing. Next steps will involve conversations with the California Department of Public Health, reviews of pertinent All Facilities Letters, and updates to CHA’s discharge reference materials.
