Meetings Aim to Improve Patient Flow from Acute to Long-Term Care

What

Hospital Council has begun meetings with members in the Fresno-Madera-Tulare-Kings Section to better understand the issues facing general acute care hospitals (GACHs) when trying to discharge a patient to long-term care. The barriers to transfer vary, but they fall broadly into issues with payers, receiving facilities, and mental health infrastructure.

Takeaway / Next Steps

Hospital Council is meeting with additional member GACHs to learn more about the unique issues each one faces and is consulting with the California Hospital Association on specific issues. The goal is to bring together a collaborative with relevant stakeholders from around the service area to improve patient movement to appropriate levels of care.
