Several of the hospitals in Santa Clara County have expressed concern about discharge challenges and have asked Hospital Council to bring together hospitals, health plans, and county services to discuss possible solutions. As our population gets older, there is a lack of skilled-nursing beds to serve the aging population in the county. In addition, hospitals are seeing younger patients who are in their 40s and 50s and need a skilled-nursing setting; however, they are too young to stay at these facilities. These challenges build upon the lack of a higher level of care for individuals who are seeking behavioral health services.
